This form is to be completed by health professionals. Our midwifery at home service offers 1-5 home visits by a registered midwife tailored to individual needs, in the early days after hospital discharge. Early discharge is possible if there are no medical complications for the women or her baby, she is feeding independently, and her primary care provider approves.
